What do they call the price list for hospital services?

What do they call the price list for hospital services? I forgot what it's called. It's a catalog of surgeries and other things that a hospital does for patients. It's basically so hospital employees know how much to bill the customer (insurance company) for.

1. they are item numbers,and they use a medical reference schedule.some doctors bulk bill,others charge the medical schedule price,others charge above the fee.i hope this helps you find the right word your looking for,it may jog something.i used to work for a health insurance commission,a long time ago tho.
