1. you could get a bumper at a wholesale GM dealer for cheap. My bumper was 100 bucks. Gm makes more parts than they need. most bumpers are univeral
2. I'd say, $300-$400. Take the picture to three body shops, get written estimate. They will probably tell you to "bring it in" though.
